Independent Contractor Disclosures Flexx Workforce of Louisiana LLC

It is expressly agreed by the parties and acknowledged by the Independent Contractor that:

Compensation:The Independent Contractor shall be compensated at a flat rate for the labor services performed pursuant to the terms of this agreement.
Independence:Flexx Workforce of Louisiana LLC shall have no right of control or direction over the manner, means, or methods the Independent Contractor uses to fulfill the services described herein.
Provision of Equipment and Supplies:The Independent Contractor shall supply, at their sole cost and expense, all labor, materials, tools, supplies, and equipment necessary to complete the contracted services. Flexx Workforce of Louisiana LLC shall bear no responsibility for any such expenses.
Authority:The Independent Contractor shall have no authority to bind or enter into agreements on behalf of Flexx Workforce of Louisiana LLC. No partnership, joint venture, or employment relationship is created by this agreement.
Compliance with Laws:The Independent Contractor certifies compliance with all applicable federal, state, and local laws, including but not limited to obtaining all necessary business licenses, permits, and registrations required to perform the services described herein.
Insurance:The Independent Contractor shall be solely responsible for procuring and maintaining any and all insurance coverage necessary to perform the services under this agreement.
At-Will Engagement:The Independent Contractor acknowledges that their engagement with Flexx Workforce of Louisiana LLC is at-will and may be terminated by either party at any time, with or without cause or advance notice.
No Benefits Provided:The Independent Contractor understands that they are not eligible for, and will not receive, any employee benefits from Flexx Workforce of Louisiana LLC, including but not limited to health insurance, retirement benefits, workers’ compensation, or unemployment insurance.
Confidentiality:The Independent Contractor agrees to maintain the confidentiality of Flexx Workforce of Louisiana LLC’s operations, clients, and business practices and shall not disclose any confidential information to third parties without written authorization.
Drug-Free Workplace Policy:The Independent Contractor acknowledges that Flexx Workforce of Louisiana LLC maintains a drug-free workplace and agrees to comply with all related safety and conduct standards.
Accuracy of Application Information:The Independent Contractor certifies that all information provided in the application process is true, complete, and accurate to the best of their knowledge. Any false, misleading, or omitted information may result in disqualification or immediate termination of the contractual relationship.
Payment and Tax Responsibilities:The Independent Contractor shall be compensated by Flexx Workforce of Louisiana LLC and will receive an Internal Revenue Service Form 1099 for all payments made. Flexx Workforce of Louisiana LLC will not withhold or pay any federal, state, or local payroll taxes on behalf of the Independent Contractor or any employees of the Independent Contractor.
Tax Liabilities:The Independent Contractor is solely responsible for the payment of all federal, state, and local taxes arising from payments received and agrees to indemnify Flexx Workforce of Louisiana LLC from any claims, penalties, or liabilities related to their tax obligations.
Garnishment Administrative Fees:In the event that Flexx Workforce of Louisiana LLC is contacted by any state or federal agency regarding a wage garnishment, an administrative fee of $3.00 per pay period will be deducted from the Independent Contractor’s payments to cover administrative processing costs.
